I am the owner of this entry and it is my original work. I am submitting this work for review. While I understand that I retain all rights to ownership of the work, I hereby give the Shriekfest Film Festival permission to feature, advertise, display, in whole or in part, the work titled and described above. I also understand that my entry is valid only upon completion of the following conditions: (1) receipt of a copy of the work and this completed signed form, (2) the processing of the entry fee by the festival organization, and (3) when the work is found to be original and owned by me or those I represent. I understand that my submission is in no way a guarantee of acceptance into Shriekfest. No one from Shriekfest or its sponsors has guaranteed me admission into the festival. Step 2: Complete all applicable fields and sign the document to confirm that you are the sole owner(s)
and originator(s) of the work.
Step 3: Place the completed form, your entry fee check, a self-addressed stamped envelope (if you
would like your entry returned), and 2 (two) copies of your NTSC DVD entry or 1 (one) copy of your screenplay or original song cd in a single package for submission. OR You may email your entry form and screenplay to us at shriekfest@aol.com Screenplays must have a title page with the title and writer's name and they must be bound and not be more than 120 pages.
All accepted filmmakers will be required to supply a self addressed stamped envelope for their
screeners and applicable insurance (if needed).
Step 4: Submit package.
IMPORTANT NOTE: We are not responsible for lost, stolen, or missing entries. We would like to remind you to send a COPY of your original work, not the original work itself. Tapes, DVD's & Screenplays will not be returned unless a SASE is provided, however, we will contact you to let you know we received your film or screenplay and we will contact everyone whether you are a finalist or not. Please do not send submissions "signature required". We will email you to confirm receipt.
The festival will still be held in Los Angeles

Super shorts/Webisodes (10 min/pages & under, including credits) Regular: $25.00, Late: $35.00
Regular shorts (11 min/pages-49 min/pages, including credits) Regular: $35.00, Late: $45.00
Features (50 min/pages & up) Regular: $45.00, Late: $55.00.
These same prices apply to the screenplays, too.
We have made it a policy to NOT grant submission waivers to anyone, this way it is fair across the board.
If you would like 2 pages of script coverage by a script doctor, please add $60.00 to your entry fee.
These fees may be paid by check, money order, or travelers checks payable to: Shriekfest Film Festival
or you may pay with credit card using PayPal After you open your free account hit send money and type in this
shriekfest@aol.com. Please note: If you are paying with PayPal you must add $3.00 to your entry amount to pay for PayPal's fee.
Please have your submission postmarked by the deadline dates. Entry fees are non-refundable.
The format that will be accepted for qualification is NTSC DVD,
but festival screenings can be on most any format.
